The authors and editors are to be congratulated for producing an excellent, up-to-date resource. Working through a diagnostic conundrum, from presentation, through careful history taking, and examination, supplemented by relevant investigations, is an intellectual challenge, a form of hypothesis testing, the goal of which is to arrive at a strong probability that the null hypothesis you have formulated should be rejected.
