From my experience of more than four decades, I can emphatically say that homoeopathy as a system of therapeutics is complete in every respect. It has enough tools and methods to meet all the problems in its own way, of\\ncourse only when its laws are practiced in their purity and entirety. Homoeopathy fails only if they are forced outside beyond their real sphere or if they are abused, perverted, emasculated or molded in order to fulfill the unsound demands of the society or to gratify the selfish interests of the Materialists. The sole objective and the only mission of a true homoeopath is to cure the sick. And for achieving this noble goal, he first need to perceive the SICKNESS - the disorder in its totality from which the sick individual is suffering and then to decide appropriate remedial measures including medicinal agents which corroborates the disease picture. In order to fulfill this task diligently, every physician is expected to undertake the following exercises with all its seriousness and confidence. They are : 1. Case Taking Recording of the life history including the present complaints 2. Case Analysis Anamnesis 3. Case Synthesis Totality of Symptom 4. Repertorisation for selecting a group of indicated remedies. 5. Similimum selection from Materia Medica. Master Hahnemann had no readymade case taking proforma. He collected the symptoms according to the case. However, Dr Kent has outlined the vital points for collecting symptoms. In this treatise I have presented a detail yet short proforma to receive the case. It contains all the essential points necessary for a homoeopath to conceive the true image of sickness in the patient.
